Scheduling, Forms & Payment
New clients will be asked for some general information before an appointment is scheduled. Understandably, most clients have questions about therapy and the process. Feel free to call me at (954) 642-6776 for information and to schedule an appointment. You can also simply fill out the request form below with your full contact information and I will be in touch with you as soon as possible.
For your convenience, you may access the forms needed for your initial appointment once you are scheduled. Please fill, print and bring to your appointment. This allows us to spend the entire session time more efficiently.
Payment is due at the time of your appointment. I accept cash, check and credit cards in the office. If you prefer to pay for your session prior to your scheduled appointment you may do so by clicking the link below (you do not need to have a Paypal account). If you wish to use your health insurance, please check with your insurance provider as to whether your plan offers out-of-network Mental Health benefits as I do not accept insurance. I can provide you with the receipts you will need to file for reimbursement from your insurance company. The amount you will be reimbursed will depend on your insurance plan (usually ranging from 40-90%.) Some clients, due to concerns about privacy regarding their mental health records, choose to privately pay rather than utilize their out-of-network insurance benefits.
